About the Position
RN  responsible for comprehensive assessments in the patient’s home, including the completion of OASIS using electronic documentation.
In addition to OASIS assessments, RN perform skilled nursing visits as needed and are responsible for delegation and coordination of care with LVNs and HHA team members
Must be fully skilled in lab draws, catheter placement, wound care, wound vac, and IV
Requirements
Current Texas Registered Nurse license
Current Texas driver’s license
Automobile liability insurance issued in Texas

Job Description

We are seeking a licensed and motivated Physical Therapist (PT) to join our home health team. As a PT, you will be responsible for evaluating patients, developing individualized care plans, and providing rehabilitative services in the home setting.

Key Responsibilities

Perform comprehensive patient evaluations in the home environment
Develop and implement individualized treatment plans
Monitor and document patient progress by home health regulations
Communicate effectively with physicians, nurses, caregivers, and agency staff
Educate patients and families on exercises, safety, and adaptive equipment
Ensure compliance with agency policies and applicable federal/state regulations

Qualifications

Current Physical Therapist license** in the state of TX
CPR certification (required)
One year of PT experience (home health preferred, but not required)
Valid driver’s license and reliable transportation
EMR proficiency
Strong communication, documentation, and time-management skills
